“Mom let’s race”. Why wouldn’t we join in to race? It’s what we have been training for since before our children were born. When I say train, I mean pelvic floor training. My little fella was getting all psyched ready to run, while I, excuse the pun... was squeezing in a quick set of pelvic floor exercises before starting.

Our pelvic floor plays a really important role during pregnancy and labour. Firstly it supports the weight of our growing baby, with our muscles becoming more challenged with the heavier our baby gets. Affected by pregnancy hormones preparing you for childbirth and later on when labour begins your pelvic floor certainly is put through its paces.

After 9 months supporting your body and the necessary stretching required to have your baby your pelvic floor muscles are understandably weaker after birth. You all know the phrase “I laughed so hard I almost peed my pants” well it’s applicable to more ladies than you would think.

Research by Always Discreet showed two thirds of women in the UK (67%) have experienced bladder leaks including much more than half (64%) of 18-24 year olds - so much younger than I would have thought.

Studies show the practice of yoga for 1-2-hour sessions totaling 3-5 hours per week for 6-8 weeks may improve pain and quality of life for those with chronic pelvic pain.

Studies did not indicate type of yoga or if shorter sessions would also be as effective.

But it may be worth a try!

1.🔸Beba água: É importante lembrar que a quantidade de água  adequada para cada pessoas é diferente (35ml de água para cada Kg de peso corporal).
2.🔸Evite café, álcool, refrigerantes e cigarros: Essas substâncias podem irritar a bexiga e aumentar excessivamente a frequência urinária.

3. 🔸Evite passar mais de 3 horas sem fazer xixi: isso pode alterar a capcidade e a sensibilidade da sua bexiga.

4.🔸Faça xixi após as relações sexuais: evita que bacterias e fungos da região vaginal e anal permanecam na uretra e causem infecções.

5.🔸Regule seu intestino: a constipação é um fator de risco para a incontinência urinária.

6. 🔸 Ao urinar, esvazie a bexiga completamente: o resíduo de xixi pode causar infeções urinárias.

7. 🔸Ao usar o papel higiênico, o movimento deve ser de frente para trás, de modo contrário você  pode trazer bactérias do ânus para a vagina e uretra.

8.🔸Não faça força na barriga para iniciar a saída da urina, apenas sente e relaxe os musculos pélvicos.

9.🔸Evite usar absorvente e protetor diário: eles abafam a região, favorecendo a infecções.

10.🔸Faça exercícios para os músculos do assoalho pélvico: adequam a força e o tônus muscular evitando e tratanto problemas urinários.

Hunter had to be sedated today and have his booty cleaned up, some of the tissue on his penis has died and we are now considering a PU (perineal urethrostomy) again in hopes to eliminate this issue all together. 
We should know more soon ♡ I hope that he will be able to go to his birthday part on the 31st!

Como já falado no post de incontinência urinária, o tratamento convencional recomenda que sejam realizadas mudanças no estilo de vida, o controle miccional, o treinamento da bexiga, treinamento da musculatura do assoalho pélvico (TMAP), estimulação elétrica, estimulação magnética e estimulação do nervo tibial posterior.

O TMAP é o padrão ouro de tratamento, pois apresenta melhor eficácia na melhora do sintoma da IU quando comparados com outras técnicas. 
O treinamento pode aumentar o volume muscular, formando um suporte estrutural para a bexiga e a uretra e assim uma contração rápida e forte da MAP durante o aumento da pressão intra-abdominal pode prevenir a descida uretral.

Uma vez que os resultados do TMAP são específicos para o tipo de contração que está sendo treinada, a situação de treinamento deve refletir a função na qual o efeito é desejado. 
A MAP é constituído por 70% de fibras do tipo I (contração lenta), e 30% das fibras tipo II (contração rápida). As fibras musculares do tipo I são resistentes à fadiga, pois tem capacidade de realizar um movimento repetidamente por longo período de tempo. Já as fibras do tipo II produzem força, velocidade máxima de encurtamento e potência, porém fadigam rápido. 
Cerca de 30% das mulheres não conseguem contrair corretamente a MAP na sua 1ª consulta. O erro mais comum é fazer uma manobra invertida, ao invés de contrair o esfíncter (para cima e para dentro), as mulheres realizam a manobra de valsalva (de expulsão) e pode piorar a incontinência por aumentar a pressão intra-abdominal e o uso associado da musculatura acessória como glúteos, adutores de quadril e músculos abdominais, pode mascarar a força da contração da MAP, de tal forma que a pessoa acredita que está realizando uma forte contração, mas a MAPs está contraindo de forma insuficiente.

THE PELVIC FLOOR 💁

This week I had an asthma patient in my clinic with a sharp rise in her breathlessness and reduction in exercise tolerance over the last 3 weeks. 😷

When her lung function came back stable and she had minimal other respiratory symptoms I had to dig a little deeper 😯

Upon palpation of her stomach (an integral part of any breathing assessment) I found significant over-activation of all her stomach mucles including rectus abdominals and her obliques.... Interesting. 
I asked:
"have you been doing a lot of stomach evercises" 
She replied:
"I have been trying to strengthen my pelvic floor"

I asked
"for how long have you been doing these exercises?" She replied:
"every hour for the last 3 weeks"

I yelled:
A-HAH it was the asthma after all!! But but what? 
Up to 70% of women with lung disease have stress urinary incontinecne caused by the repititive loading of excessive coughing in lung disease. 😫

Therefore I reccommend all women with lung disease start pelvic floor strenthening however please dont just 'google it' like my patient above.. see a professional womens health physiotherapist to get it done right! 👍

Am I ready to run? 🏃‍♀️ Whether you are returning from an injury or just had a baby, it’s important to make sure your body is capable of handling the demands of running before you jump back into it full swing. 🏃🏻‍♀️ One of the things I look at if someone is wanting to run is their single leg stability with hopping and bounding. Running is essentially sequential single leg mini squats, and you want to be able to transfer load through your lower extremity as efficiently as possible. 🏃🏽‍♀️ If you compare the videos, you’ll notice in the first 2 my knee is diving in, my pelvis drops, and I have a hard time just getting my balance. If someone has this same strategy every time they land in a run, it has the potential to lead to foot, knee, hip, or low back pain. In the last 2 videos, I’m able to actually control my landing and maintain the position of my foot, knee, and pelvis. This translates to improved load transfer and more effective running mechanics. 🏃🏽‍♀️ In addition, if you are experiencing pelvic heaviness, pressure, or are leaking with something like this, you would benefit from a consultation with a pelvic physical therapist to evaluate how ready your pelvic floor is for something as demanding as running. 🏃🏿‍♀️

🌿So lesson 1/4 is the need for a baseline level of strength, yes in your pelvic floor, but also in the muscles you are using to take off and land in whatever particular jump you plan on doing.⁠⠀

🌿Eccentric strength is the focus here.  Landing from a jump exerts more force on the body (and the bladder onto the pelvic floor) than the weight of your body.  Something about physics, acceleration, and gravity...I'm not into math but it's more.⁠⠀
⁠⠀
🌿So if we train our muscles to decelerate the landing properly, that force will theoretically be spread out to your ankles, knees, and booty rather than just jarring your bladder and requiring a lot of force closure of your pelvic muscles.⁠⠀
⁠⠀
This example is landing a jumping lunge!⁠⠀
1️⃣Fast up with coordination of breath⁠⠀
2️⃣Slow down through full range of motion⁠⠀
- this burns zee muscles = getting stronger⁠⠀

This little guy right here means the world to me! He is my youngest child out of 3 kids, and he has brought such joy to my life. My pregnancy with him was pretty uneventful, but I was induced 1 week early because of my history of having large babies and quick deliveries. My 2nd child weighed 10lbs. 2oz!! I had all of my babies vaginally which has taken a toll on my small framed body.

A year and a half after my youngest child was born, I was diagnosed with a bladder prolapse. I was only 32 years old and I was devastated! My GYN told me that my options were to wear a pessary or have corrective surgery. If you don't know what a pessary is, its a medical device that's inserted into the vagina to support your tissues. There was no way at 32 years old that I was wearing a pessary, so I opted for surgery.

Looking back, I am not surprised about what happened to me. I had been struggling with stress incontinence for a long time. I never thought to do anything about it though. 
Now I know how important pelvic health is, especially after having children and what I went through.

Can you relate to my story? Do you have signs or symptoms of a weak pelvic floor such as urinary incontinence or tampons that dislode or fall out? 
I have good news! It's possible to strengthen your core and pelvic floor muscles to prevent a prolapse like I had. You can do kegel exercises, but there is a much more efficient way of doing them that I want to share with you!

I just received an interactive pelvic floor training system and device. I tried it out for the first time today, and I am very impressed! I love that that it can fit any sized woman, it gives you automatic feedback, and it challenges you to get stronger with the app and interactive workouts.

“I can’t feel my pelvic floor move” - all my patients in the beginning

What I have learned from @posturalrestoration Is that if you can’t “feel” something then something else is probably “tight”, and needing some inhibition. If a tight back or tight neck is talking so loud it would make sense,  to me anyways, that PF would get ignored and your body would struggle to perceive its motion/function. This aligns with the statistics that show low back pain and pelvic floor dysfunction more often than not go hand in hand. .

Today I experimented with a little back inhibition that fit into #momlife at the park and felt fantastic on low back and allowed me to feel my #pelvicfloor move with my breath. Obviously not a #pregnancyexercise but versions of this in pregnancy could feel fantastic too! .

I’ve seen @harris_ssp Use versions of this in his fitness classes at @sandhillssportsperformance makes me want to join in! .

COFFEE!!! As moms we neeeed it to get by. This mug was given to me by a sweet patient of mine after the birth of my 3rd child. 😂☕️ Coffee is a lifeline. But it's also worth knowing that if you find yourself having a lot of urgency to pee or that you have to pee every. single. hour... it could be that cup of joe. Coffee is sadly a bladder irritant and can upset your bladder. It can even cause you to leak! So replace that second cup with water or cut it out altogether for a week and see if your bladder thanks you. 😉

COFFEE!!! As moms we neeeed it to get by. This mug was given to me by a sweet patient of mine after the birth of my 3rd child. 😂☕️ Coffee is a lifeline. But it's also worth knowing that if you find yourself having a lot of urgency to pee or that you have to pee every. single. hour... it could be that cup of joe. Coffee is sadly a bladder irritant and can upset your bladder. It can even cause you to leak! So replace that second cup with water or cut it out altogether for a week and see if your bladder thanks you. 😉
